Adult and Youth Crisis Services
Frequently unpredictable,
crises can occur anytime. The availability of
immediate support often makes the critical difference.
Woodland Centers' 24-hour
toll-free crisis hotline,
1-800-992-1716, gives callers information about
available services and offers immediate options for
emergencies. Walk-in service also is available.
Both youth and adult
residential crisis beds serve clients who are in acute
distress and require stabilization, evaluation, and
appropriate referral into additional outpatient or
inpatient care. The youth component is licensed as
a Rule 5 facility and the adult component is licensed as
a Rule 36 facility.
The combination of crisis
hotline, walk-in service, urgent care, professional
on-call, and crisis bed availability provide a continuum
of access to meet the emergent behavioral health needs
of individuals in West Central Minnesota.

Crisis
Services
Service Description
Woodland Centers has
been awarded a grant to offer mobile crisis services to
youth. The service is available in Kandiyohi, Meeker,
Swift, Renville, Chippewa and LQP counties. Mobile crisis
response to the site of the crisis will be available
Monday through Friday 12 noon-8pm. Additional service
hours may be added as need and resources permit.
Youth Mobile Crisis (YMC)
services are face-to-face, short-term mental health
services initiated during a crisis to help a child return
to baseline level of functioning. Children’s crisis
response services are provided on-site by a mobile crisis
response team outside of urgent care, inpatient or
outpatient hospital settings.
On site services are provided by one or two Mental Health
Practitioners. A licensed mental health professional is
always available to the Practitioners by telephone for
consultation and supervision.
Crisis
Screening
When someone calls the
YMC service, the staff person will conduct a screening of
the potential crisis situation. The screening will: gather
information; determine whether a crisis situation exists;
identify the parties involved; and determine an
appropriate response.
Circumstances when mobile
response might be indicated:
1. Child expressing thoughts of self-harm or harm to
others
2.
Escalation of potentially out of control behavior
3.
Acute parent child conflict
Other
matters considered to determine mobile response:
1. Parent/guardian consent is required for the
service to be delivered.
2.
Parent/guardian has indicated desire for the child to
remain at home.
3.
Supervising mental health professional approval is
required.
4.
Two staff are required for mobile crisis assessment.
5.
Depending on circumstances, two staff may not be necessary
for intervention or stabilization.
6.
Staff will not/cannot perform physical holds or otherwise
intervene physically.
7.
Staff will not/cannot provide transportation.
Crisis
Assessment
If
a mobile response is initiated, upon arrival to the site
of the crisis the Practitioner will conduct a face-to-face
assessment. The crisis assessment aims to provide
immediate intervention to assist in relief of distress
based on a determination that the child’s behavior is a
serious deviation from the child’s baseline level of
functioning including:

Life situation and sources of stress;
•
Symptoms, risk behaviors, and mental
health problems;
•
Strengths and vulnerabilities;
•
Cultural considerations;
•
Support network; and
•
Functioning.
Crisis Intervention
Crisis interventions are
face-to-face, short-term intensive mental health services
started during a mental health crisis or emergency to help
the youth
•
Cope with immediate stressors and lessen suffering;
• Identify and use
available resources and the youth’s strengths;
• Avoid unnecessary
hospitalization and loss of independent living;
• Develop action
plans; and
• Begin to return to
the youth to baseline level of functioning.
• Recommend needed
services, including crisis stabilization;
• Refer to appropriate
local resources, such as the county social services
agency, mental health
services, local law enforcement;
• If the recipient has
a case manager, coordinate the planning of other services
with the case manager.
Crisis
Stabilization
Crisis stabilization
services are mental health services provided to a youth
after crisis intervention to help the obtain a functional
level as it was before the crisis. Stabilization services
are provided in the community and are based on the crisis
assessment and intervention treatment plan.
